Job Description

An established Home Care organization is looking for a personable and detail-oriented Intake Specialist to join their team in Brooklyn. The organization is a fully licensed and bonded home health care agency serving Brooklyn, Manhattan, Queens, The Bronx, and Staten Island, as well as a Fiscal Intermediary in most New York counties, and is the home care agency of choice for thousands of families. The ideal candidate will be the first point of contact for prospective clients and their families, guiding them through the intake process with care and professionalism.

Responsibilities:

  • Client Onboarding:
    • Respond to inquiries from prospective clients via phone, email, or in-person visits.
    • Explain home care services, eligibility requirements, and how our agency supports the Frum community’s unique needs.
    • Schedule assessments and follow up to ensure a smooth onboarding process.
  • Documentation & Compliance:
    • Collect and verify necessary client information and documentation.
    • Ensure all forms, consents, and contracts are accurately completed and filed.
    • Maintain compliance with state regulations and agency policies.
  • Communication & Coordination:
    • Liaise with clients, families, and internal teams to coordinate services.
    • Address questions and concerns with empathy and professionalism.
    • Collaborate with case managers, caregivers, and administrative staff to create customized care plans.
  • Community Engagement:
    • Act as a bridge between the agency and the Frum community.
    • Attend community events or shul meetings to promote awareness of home care services.

Qualifications:

  • Prior experience in healthcare, intake coordination, or customer service, preferably with the frum community.
  • Strong communication and interpersonal skills.
  • Proficiency in Microsoft Office, basic data entry, and electronic health records (EHR) systems.
  • Detail-oriented with excellent organizational skills.
  • Ability to handle sensitive information with discretion and confidentiality.
