Job Description

An established Home Care organization is looking for an experienced Intake Specialist to join their expanding intake team for their Brooklyn location. The organization is a fully licensed and bonded home health care agency serving Brooklyn, Manhattan, Queens, The Bronx, and Staten Island, as well as a Fiscal Intermediary in most New York counties, and is the home care agency of choice for thousands of families. Responsibilities:

  • Cultivate and manage relationships with prospective clients
  • Monitor pending cases for a specified time frame
  • Follow up on the intake cases and load and assist patients/their families with the appropriate services available to them
  • Contact patient PCP for necessary documents and follow up where necessary
  • Send referrals to MLTC’s and follow up on status and update appropriate parties.
  • Send the necessary documents to Patient Assistants and set up appointments for them to register
  • Intake
    • Answer calls, emails and messages
    • Explain program
    • Check Medicaid/ePaces
    • Mail CDPAP package
    • Get PCP docs
    • Send MLTC referral to MLTC
    • Input on computer
  • Follow up with intake clients
  • Follow up with intake PAs
  • Follow up with MLTC
  • Manage MLTCS & CDPAP form communication
  • Other duties as assigned

Qualifications:

  • 1 year of healthcare experience required
  • No degree needed
  • Basic computer skills are necessary
  • Must have excellent interpersonal and communication skills both verbally and written and have a high degree of independent problem-solving capability.
  • Must demonstrate strong attention to detail. 
  • Must have excellent time management, organizational, and communication skills and be a team player who is self-motivated.
  • Must be an assertive individual who is able to work in a fast-paced environment
