Job Description
A well-established elder care services provider is seeking a compassionate and organized Case Manager to support older adults in maintaining their independence and quality of life. This role involves assessing client needs, developing and managing individualized care plans, coordinating with service providers, and ensuring ongoing support through regular monitoring. The ideal candidate is experienced in case management, familiar with community resources, and comfortable working independently in the field. A valid driver’s license and reliable transportation are required, as quarterly home visits and travel between client locations are essential to the position.
Responsibilities:
- Conduct initial and ongoing assessments to determine client needs and strengths.
- Develop, implement, and adjust individualized care plans.
- Coordinate services with healthcare providers, social agencies, and family members.
- Perform regular home visits and wellness checks.
- Advocate for clients and assist them in accessing needed resources.
- Maintain thorough, timely documentation in compliance with internal and external requirements.
- Monitor client progress and address changes in condition or service needs.
Qualifications:
- Associates or Bachelor’s degree required
- Minimum 1-2 years of case management or social service experience preferred, preferably with older adults.
- Strong communication, time management, and organizational skills.
- Proficient with Microsoft Office and case management systems.
- Valid driver’s license and access to a reliable vehicle.
- Bilingual candidates (Spanish or Russian) is a plus.
- Preferred Qualifications:
- Case Manager Certification (CCM or ACM).
- Experience with crisis intervention and elder advocacy.
- Familiarity with elder care laws, resources, and regulations.
