Field Nurse Supervisor

Job Description

A compassionate LHCSA company dedicated to delivering high-quality home care services is seeking a Field Nurse Supervisor to join their team. They are committed to helping clients maintain dignity, independence, and quality of life in the comfort of their homes. The Field Nurse Supervisor will oversee a team of field nurses, ensuring exceptional patient care and regulatory compliance. The role offers a balance of fieldwork and office-based responsibilities, managing a patient load of approximately 50 clients across Brooklyn and Queens.

Responsibilities:

• Supervise, support, and train a team of field nurses
• Conduct initial assessments, supervisory visits, and re-assessments in patients’ homes
• Ensure clinical documentation is accurate, complete, and compliant with agency and DOH guidelines
• Work closely with the team to maintain timely compliance paperwork
• Serve as a liaison between field staff, patients, families, and administrative staff

Requirements:

• Current NYS Registered Nurse (RN) license
• Minimum 2 years of home care experience, specifically within a LHCSA
• Strong clinical assessment and documentation skills
• Comfortable traveling throughout Brooklyn and Queens for home visits
• Supervisory or leadership experience is a plus
