Job Description

An established Home Care organization is looking for a Patient Relations Representative for their Brooklyn location. The organization is a fully licensed and bonded home health care agency serving Brooklyn, Manhattan, Queens, The Bronx, and Staten Island, as well as a Fiscal Intermediary in most New York counties, and is the home care agency of choice for thousands of families. This role involves acting as a liaison between patients, their families, and healthcare providers to address concerns, enhance communication, and improve overall patient satisfaction.

Responsibilities:

  • Patient Advocacy: Serve as the primary point of contact for patients and their families, addressing concerns, complaints, and requests with empathy and professionalism.
  • Communication: Facilitate effective communication between patients, families, and homecare providers to ensure understanding of care plans, procedures, and policies.
  • Record Keeping: Maintain accurate and detailed records of patient interactions, complaints, and resolutions in accordance with company policies and confidentiality standards.
  • Quality Improvement: Participate in quality improvement initiatives and contribute to the development of policies and procedures aimed at enhancing patient satisfaction and care in the homecare environment.
  • Support Services Coordination: Assist patients and families in accessing additional services such as financial counseling, social work, and support groups.

Qualifications:

  • High school diploma or equivalent required; Associate’s degree or higher in a related field preferred.
  • Minimum of 2 years of experience in a healthcare setting, preferably in homecare or patient relations.
  • Strong problem-solving and conflict resolution abilities.
  • Excellent interpersonal and communication skills.
  • Proficiency in Microsoft Office Suite and familiarity with electronic health record systems.
  • Ability to handle sensitive information with discretion and maintain confidentiality.
