Job Description
A well-established, growing primary care clinic in Fresno, CA, supported by a rapidly growing healthcare management organization, is seeking an MD (preferred) or NP/PA with strong experience in primary care for geriatric or senior populations. This clinic currently operates with two physicians and serves a primarily HMO patient base, including annual wellness visits and chronic disease management. The ideal candidate is patient-focused, efficient, and comfortable managing a high-volume outpatient panel of 20–30 patients per day in a structured, well-supported clinical environment.
Responsibilities
- Provide comprehensive primary care to an adult and geriatric patient population.
- Deliver in-clinic primary care, including evaluations, follow-ups, preventive care, and chronic disease management
- Collaborate closely with clinic physicians, medical assistants, case managers, and operational staff.
- Deliver high-quality, evidence-based care aligned with HMO standards, preventive guidelines, and organizational protocols.
- Maintain accurate and timely documentation within the EMR system.
- Coordinate care with specialists, hospitals, and care-management programs as needed.
- Participate in quality initiatives, patient-outcome programs, and performance metrics.
- Contribute to a positive and collaborative clinic culture focused on senior care excellence.

Requirements
- Active, unrestricted California license as an MD/DO, NP, or PA; Board Certified/Eligible (MD/DO) or nationally certified (NP/PA).
- 2-4 years clinical experience in primary care, internal medicine, family medicine, or geriatric/senior care, including management of chronic conditions.
- Experience with Medicare Advantage, HCC coding, preventive care, and Annual Wellness Visits (preferred).
- Ability to manage a high-volume outpatient schedule of 20–30 patients per day while maintaining high-quality, patient-centered care.
- Proficiency with EMR systems and commitment to accurate, timely, and compliant documentation.
- Strong communication skills with the ability to educate patients and collaborate effectively with interdisciplinary clinical teams.

Salary: MD $220–280K; NP/PA $130–180K.
