Job Description
An established Home Care organization is looking for a full-time Coordinator fluent in Spanish to join their team in Brooklyn. The organization is a fully licensed and bonded home health care agency serving Brooklyn, Manhattan, Queens, The Bronx, and Staten Island, as well as a Fiscal Intermediary in most New York counties, and is the home care agency of choice for thousands of families. This role will focus on organizing and managing day-to-day activities, supporting home care staff, and ensuring that our clients receive the highest standard of care.
Responsibilities:
- Coordinate daily scheduling and assignments for home care staff.
- Communicate effectively with clients, families, and staff to ensure service needs are met.
- Handle inquiries, resolve scheduling issues, and provide follow-up on client care.
- Maintain up-to-date client and caregiver records in compliance with LHCSA regulations.
- Use HHAeXchange software to manage scheduling, documentation, and client data efficiently.
- Collaborate with other team members to streamline operations and improve service quality.
Qualifications:
- Prior experience in healthcare coordination a must.
- Knowledge of HHAeXchange
- Must be able to speak Spanish
- Strong organizational and multitasking abilities.
- Excellent communication skills.
- Ability to work well independently and as part of a team.
